Registration will be on Friday October 8th at Forestville Elementary School. Registration will be open from 5:30 - 9:30 PM. For Registration please make sure you have the following with you.

1) Bring your original roster and 2 Copies of your original team roster to registration. We will not have access to a copy machine. You will need to bring copies with you.

2) If you are using guest players - please add the guest players to the COPY of your rosters. Do not modify your original roster.

3) Bring your player passes to registration. These will be checked at registration against your roster. Each player will either need a recreational pass from their club or a travel player pass from another team.

4) Each player will also need a medical release form. The medical release form that you have for this current season for each player is sufficient. If you are using a guest player, the guest player will need a form signed by the parent or guardian. You can access the VYSA Medical Form by clicking here.

5) Please fill out and bring the Weekend Contact Form. This form can also be found on the tournament website under Forms or by clicking here. We will use this form to contact teams in the event we need to communicate with you.
